VIBRANT SOUNDBRIDGE IN ATRESIA OF EAR: SIMULTANEOUS BILATERAL SURGERY

Abstract:

OBJECTIVES: To describe the surgical technique and the auditory result of auditory bilateral simultaneous implantation of prosthesis of average ear Vibrant SoundBridge ® in patient with atresia of ear and microtia. CASE REPORT D.C.N., masculine, 18 years, and carrier of bilateral congenital atresia of ear, with aero-osseous moderate conductive deficiency auditory, gap of 50db. Not adapted satisfactorily with which had vibratory arc to the discomfort generated for the pressure exerted in mastoid. Ahead of this, auditory prosthesis of average ear was considered one as alternative - the Vibrant SoundBridge®. The Computerized Tomography of bilateral secular bones showed presence of supply-structure of stirrup, casting hammer and two-horned anvil in aerated attic and face nerve in normal position. Internal ear without alterations with patent windows of the choclea. Submitted to the simultaneous bilateral surgery with timpanomastoidectomy and ample opening of the attic and withdrawal of atrophic bone. Withdrawal of fixed hammer and two-horned anvil and exposition of the supply-structure of the stirrup, furniture on both sides. The bobbin of the internal unit was connected in prosthesis of titanium specific for this end (PORP) and incased in the stirrup. In the comparative audiogram, free field, average auditory thresholds of 20dB in the frequencies of 500Hz 4000Hz, with great familiar satisfaction of the patient and. CONCLUSION: Even so the surgery in atrtic ear is complex, the restoration of the implantable hearing with prosthesis of ear measured is efficient and in favorable anatomical conditions can the same be carried through bilaterally in surgical time.
