SÍNDROME DE SAMTER

Abstract:

INTRODUCTION: The association of Poliposis Nasal-sinusal (PNS), Intolerance to aspirin, and Asthma initially was observed by Widals et al. (1922), and later for Sumter (1967). The occurrence of the asthma generally precedes the appearance of the polyps and the aspirin manifest intolerance mainly for nasal blockage and bronchospasm. The objective since work is to tell a case of PNS of difficult treatment, associate has intolerance to aspirin and asthma. CASE REPORT: B.A.G.P., 56 years, feminine, looked to the clinic of otolaryngologist of the Naval Marcílio Dias Hospital complaining of nasal blockage of long date, associated the episodes of epistaxe. Diagnosised with asthma in the infancy, it had intolerance the anti-inflammatory AAS and other non-steroidal. The previous rhinoscopy presented polypoid degeneration in nasal tanks. Computerized Tomography of paranasal sinus was become fulfilled where veiling of the frontal sinus was evidenced, maxillary, ethmoidal and esphenoidal. Moreover, complete blockage of the ostia-meatais complexes and increase of the volume of soft parts in nasal tanks. The patient was submitted to the nasal polypectomy with antrostomy to maxillary intranasal bilateral, bilateral ethmoidectomy and bilateral frontal sinusotomy, evolving without occurrences in the postoperative one. It kept ambulatorialy accompaniment and treatment with nasal topical corticoid. Passed three months of the surgery, the patient returned to the clinic with complaints of nasal blockage. In the physical examination return of the polypoid degeneration was inquired, confirmed in new Computerized Tomography, where veiling of all was diagnosised the sinus of the face. Quarrel: Generally the PNS cases associates to the Syndrome of Samter possess difficult treatment. The return uses to be precocious obstante the surgical and clinical treatment.
