TEMPORAL BONE FRACTURE WITH DESARTICULATION OF THE OSSICULAR CHAIN
FRATURA DO OSSO TEMPORAL COM DESARTICULAÇÃO DA CADEIA OSSICULAR

Abstract:

AMS, 36 years old, biker, complaining of hearing loss on the right that started 90 days after being hit. She denied other auditory symptoms and dizziness. At the time, was seen in a general hospital with othorrea, headache and hearing loss. Computed tomography of the date of the accident showed mixed fracture of the temporal bone and mastoid opacification. Hospital remained under observation for 03 days and released without further recommendations. Physical examination: oroscopy and rhinoscopy without changes, otoscopy revealed intact tympanic membranes, normal transparent, the handle of the malleus present on both sides and viewing incus long arm bilaterally. Rinne positive and negative in the left ear right ear. Weber lateralized to the right. Audiometric tests: normal hearing in left ear and moderate mixed hearing loss / marked in the right ear. Type A tympanogram in the left ear and Ad in right ear. Staple muscles reflexes present bilaterally. Discrimination normal in both ears. Computerized Tomography of the mastoid: no change. Diagnostic hypothesis: disarticulation of the ossicular chain to the right. Conduct: patient underwent exploratory tympanotomy under the microscope to assess the middle ear. Incudo disarticulation had no bone erosion-stapedial, stapes present and intact. It was subjected to interposition of the ossicular chain cartilaginous graft tragus, which is placed between the lenticular process of the anvil and head of the stirrup, fixation of reinforcement with fibrin glue. Patient reported significant improvement in hearing. Audiometric testing within 30 days after surgery: normal hearing in left ear and mild mixed hearing loss in right ear, type A tympanogram in both ears. Stapedial reflex present bilaterally.
