SUDDEN BILATERAL DEAFNESS: POSSIBLE ASSOCIATION WITH PURPURA IDIOPATHIC THROMBOCYTOPENIC: CASE REPORT

Abstract:

OBJECTIVE: Sudden Deafness presents as a sensorineural hearing loss greater than or equal to 30 dB in three contiguous frequencies and installed within 72 hours. In general, it is idiopathic, may be associated with autoimmune disease, viral infections and microcirculatory. There is disorder is aimed at reporting a case of bilateral sudden hearing loss associated with purpura idiopathic thrombocytopenic (PTI). CASE REPORT: Female patient 46 years old, initially treated by the clinic with complaints of hematuria, bleeding in the gastrointestinal tract, myalgia major lower limb, presence of petechiae and scattered ecchymosis by body. Without complaints. Evolved with the need for intubation for pulmonary hemorrhage. It was requested Otolaryngological examination was requested after evolve with significant bilateral hearing loss. Otoscopy showed narrowing of bilateral external auditory canal with normal appearance of tympanic membranes, and impedance audiometry showed sensorineural hearing loss of moderate to deep right ear and severe to profound in the left ear, curve type A in both ears, compatible with sudden hearing loss, maintenance of the standard audiometric tests performed on three days in the ninth to the thirtieth day of evolution, at weekly intervals. Performed pulsed dexamethasone and immunoglobulin, currently with an improvement of the PTI and monitoring on an outpatient basis. CONCLUSION: Sudden hearing loss is still a diagnostic and therapeutic challenge today. We report a rare case of bilateral involvement, and their association with purpura idiopathic thrombocytopenic.
