Abstract:

OBJECTIVE: Esthesioneuroblastoma is an uncommon malignant neoplasias that originates from neuroendocrine cells of the olfactory epithelium. The aggressive behavior, of such neoplasia, demands a surgical boarding that allows to a complete removal of the concomitant tumor to the one lesser morbity. Thus, it becomes imperative the quarrel and story of the different techniques for access esthesineuroblastoma with cranial invasion. DRAWING: The clinical case to the told being displays surgical invasive e minimum, exactly thus efficient a proposal for invasive tumors with extensive attacking of paranasal sinus and fossa previous cranial. The use of a bimodal access with the joint action of the otolaryngology teams and neurosurgery, in ours in case that, it allowed to the accomplishment of an exposed brain bifrontal without the withdrawal of the orbital rim. The tumoral resection was complete and the accompaniment in the one after delayed operative showed absence of traces of the neoplasias. The reconstitution of the imperfection osseous in cribiform plate was carried through by saw endonasal and cranial with use of piece of calot cranial, pericranial, glue, septum and nasal mucous. At the patient moment liquoric fistula or recurrence of the tumor evolves well without signals. CONCLUSION: Esthesioneuroblastomas extensive demands boarding to multisciplinar for a good result. Thus, it must be considered when feasible, the use of endoscopes associates to the transcranial access surgical resulted better visa one.
