Abstract:

The adductor spasmodic dysphonia (ASD) is one of the human most dramatic disturb of communication. The etiology of this heterogenic disturb is not yet totaly understood and has frustrated the laryngologists because no effective and definitive treatment has been proposed. This paper presents a new long-standing therapeutic possibility for the ASD. The authors applied a new surgical technique to wide the anterior comissure by interposing a cartilaginous fragment, in order to produce a little anterior triangular chink, reducting the glottic resistance during phonation and improving the voice. We present a 30 year-old female patient with a 4 year diagnostic of ASD. The proposed surgery was performed without complications and the patient followed with a satisfactory vocal quality whick remains unchanged until the moment (2 years of follow-up). This preliminary study shows that a wide variety of possibilities with new techniques based on the knowledge of laryngeal anatomy and biomechanic can be developed to improve and help patients with these social disable vocal disorders.
