TONSILLITIS OF REPETITION AND DISEASE OF BEHÇET: DISTINGUISHING DIAGNOSIS TO BE CONSIDERED
TONSILITES DE REPETIÇÃO E DOENÇA DE BEHÇET: DIAGNÓSTICO DIFERENCIAL A SER CONSIDERADO

Abstract:

OBJECTIVE: To highlight the importance of the disease of Behçet as diagnostic differential of ulcers of the verbal cavity. PRESENTATION: Of the feminine Patient CASE, 20 years, arrived at the clinic of otolaryngology with picture of tonsillitis of repetition. Surgical treatment was indicated (tonsillectomy), procedure effected without occurrences. Six months later, the patient returns with the same complaint. To the physical examination, painful verbal injuries had been noticed attacking tongue, lips and soft palate. In this consultation, it also disclosed to the simultaneous appearance of painful ulcers vulvae since the 16 years, being in gynecological accompaniment, without diagnostic briefing. In this period, it had made serological inquiry for herpes virus, anti-HIV, EBV and VDRL, all refusals, as well as diverse courses of treatment with acyclovir, benzatine and doxicicline penicillin. They had been requested, serological for autoimmunity, with positive result of FAN (1: 160, dotted core standard). Ophthalmology evaluation did not disclose signals of uveitis. The disease diagnosis of Behçet was directed for rheumatologist, receiving and initiating treatment with colchicines, with good control of the symptoms. CONCLUSION: the otolaryngologic manifestations in the rheumatic disease represent a diagnostic challenge. Patients with Behçet, can present themselves primarily to the otolaryngologists, therefore recurrent verbal ulceration is the symptom most frequent and if it confuses with other conditions, such as ulcers for trauma, recurrent mouth disease stomattatis, infections you capsize, syphilis, erythema multiforms, granulomatosis of Wegener and LES. Still, it can have overlapping of other disease as PFAPA and bacterial tonsillitis of repetition. Exactly rare, the disease of Behçet, must be considered in the distinguishing diagnosis of verbal ulcers and good anamneses can lead to the suspicion of the disease.
