OTONEUROLOGICAL FINDINGS IN 24 PEDIATRIC PATIENTS WITH MAJOR COMPLAINT OF DIZZINESS

Abstract:

OBJECTIVE: To describe the major findings in vestibular test of pediatric patients who sought care specialist complaining of dizziness. METHODS: We selected 24 patients between 5 years old and 16 years old with a chief complaint of dizziness clinic between June of 2010 and January of 2012. All patients underwent otolaryn-gologic and otoneurologic examination protocol consisting of vestibular tests and coordination, cerebelar tests, research on cranial nerves, vestibular maneuvers under video-monitoring infrared nystagmoscopy and vectoelectronystagmography, audiometry, speech audiometry and acoustic impedance with research ipsilateral and contra lateral acoustic reflexes. RESULTS: In vestibular testing and coordination in the cerebelar tests and research of cranial findings were deviations in tests Romberg, Babinski-Weil and Barre, as well as the presence of diadochokinesia. Of the total, only 8 patients (33.3%) presented with vecto- electronistagmography normal responses while 16 patients (66.6%) demonstrated hyperreflexy characteristic of irritative peripheral vestibular syndrome (SVPI). Despite the normal vectoelectronystagmography, 5 patients (62.5%) had nystagmus characteristic of benign paroxysmal positional vertigo (VPPB) vestibular maneuvers in front of infrared video nystagmoscopy, and in cases of IPVS only 6 patients (37, 5%) showed symptoms of VPPB. There were no cases of central or peripheral deficit syndromes. CONCLUSION: The findings of otoneurologic examination protocol demonstrated in the pediatric population prevalence of SVPI and were useful in the differential diagnosis and in choosing the best treatment for dizziness especially in cases with normal vec-toelectronystagmography.
