VULGARIS PEMPHIGUS IN PEDIATRIC PATIENTS: A CASE REPORT

Abstract:

Vulgaris Pemphigus is a chronic bullous disease of autoimmune nature. It is rare disease, especially in pediatric patients. It is manifested clinically by blistering flaccid, easily ruptured, crusts and erosions in areas of skin and mucous membranes, especially the oral region. The diagnosis is based on clinical and histopathological lesions. The treatment is to suppress the production of auto antibodies and is mainly based on glucocorticoid therapy. We report the case of a patient 11 years old, diagnosed with vulgaris pemphigus in July of 2011, accompanied at the Hospital of Clinics of UFMG, accompanied of the services in Ophthalmology, Dermatology, Rheumatology and Otolaryngology. Among other clinical manifestations, presented with painful oral ulcers important to weight loss due to feeding difficulties. To feed themselves, made use of xylocaine spray before meals. The Otolaryngological examination revealed the presence of ulcerative lesions in the oral mucosa, reddened and friable, which bled easily. Biopsy of the lesions showed histopathological changes consistent with pemphigus vulgaris. Established therapy with prednisone, showed significant improvement of oral lesions and nutritional capacity. The case exemplifies the oral involvement in pemphigus vulgaris. It is in most patients, the first site appearance of lesions, where it remains for about four months before they extend to other locations. It is thus reinforced the importance of otolaryngology in the recognition of lesions and early diagnosis in order to improve the prognosis for treatment and quality of life of affected patients.
