BILATERAL SUDDEN DEAFNESS FOR SYPHILIS: CASE REPORT

Abstract:

OBJECTIVES: Syphilis is a sistemic infection caused by Spirochetes Treponema Pallidum. Sudden deafness is characterized for hearing loss of sudden, sensorineural installation, of more than 30dB in at least 3 consecutive frequencies. This work aims at to tell a case of bilateral sudden deafness in patient with syphilis. CASE REPORT: E.R.T., masculine, 33 years, looked the otolaryngologic attendance complaining of followed bilateral hypoacusia of dizziness have 5 days. It denies otologic antecedents. Carried through tonal audiometric that showed bilateral deep sensorineural loss. Clinical treatment with corticosteroids in regressive dose was instituted, without getting improvement of the auditory thresholds. After one month evolved with peripheral left face paralysis. It was investigated with diverse examinations, between them magnetic resonance of internal ear and computerized tomography of skull that if they had shown normal. The alteration was found in laboratorial examinations: Reacting VDRL 1/12 and reacting sorology for syphilis (FTA-Abs). After treatment of the infectious picture, the patient remained with same the auditory levels then was indicated implantation to cochlear in left ear, carried through successfully. CONCLUSION: The told case is rare, for being a bilateral sudden deafness, of simultaneous occurrence, suggested for the examination of tonal audiometry, when showing bilateral sensorineural hearing loss above of 30 db in more than three consecutive frequencies. In such a way, importance is proven it to alert for the precocious diagnosis of the auditory commitment and for the adjusted inquiry of the pathologies that can cause the sudden deafness, and, thus, to re-echo negative in the quality of life of the patients.
