CHOCLEAR IMPLANTATION IN A PATIENT WITH BILATERAL OPEN TIMPANOMASTOIDECTOMY WITH PREPARATION OF THE CAVITY

Abstract:

OBJECTIVE: To report an implantation case to cochlear in a patient with otitis measured bilateral chronicle treated with open timpanomastoidectomy and insertion to the electrodes for the round window. CASE REPORT: V.L.V., 52 years with history of bilateral othorrea and perceived gradual hearing loss has 1 year. To the Otoscopy in the right ear it had fistula atical and atical left ear with cholesteatomatous and gill. The Audiometry presented bilateral deep sensorineural loss auditory. Computerized Tomography with signals of erosion of posterior wall of the bilateral external auditory conduit and signals of fistula of bilateral lateral semicircular channel. Patient directed for timpanomastoidectomy opened in interval of 3 months between the two ears and preparation of the right ear to receive the cochlear implantation, being confectioned remnant of muscle for reduction of the cavity and small proportional meatoplasty the cavity to favor the rank of implantation, carried through one after years the first surgery without occurrence. The surgery did not have complications up to one year after cochlear implantation. CONCLUSION: The implantation in patients with otitis measured chronicle can be fact, when it has an adequate control of the disease. Literature establishes the implantation for the way of tank measured to prevent the ear measured contaminated and the necessity of multiple surgeries before the implantation, presenting risk of comorbities. The preparation of the cavity to receive the implantation without exclusion from the ear can represent an alternative insurance stops these patients.
