INTRATYMPANIC CORTICOID WITH TOTAL RECOVERY OF THE HEARING AFTER SEVERE HEARING LOSS FOR SUDDEN DEAFNESS

Abstract:

OBJECTIVE: To after tell to a case of recovery of the hearing the normal levels intratympanic injection of corticoid after an episode of unilateral anacusis idiopathic deafness sudden causing. Story of the case: Patient of the masculine sex, 44 years, looked the emergency of otolaryngology of the Armed Forces Hospital with table of hypoacusia to the left per four days. It did not relate otalgy, othorrea, vertigo or tinnitus. The otoscopy was normal, test of positive Rinne and Weber with laterization for right. To the audiometry demonstrated normal triton average to the right with degradation in the frequencies of 3, 6 and 8 KHz and deep sensorineural loss in right ear. It initiated 1mg/kg/day use of prednisolone per 10 days, when returned and carried through new audiometry without changes. It was opted then to carrying through corticoid intratympanic with dexametasone 4mg, three applications with interval of 48 hours between them. It was telling after improves gradual of the hearing, with audiometry of 24 days after with recovery of the hearing for normal levels. CONCLUSION: The intratympanic corticotherapy still is considered controversy in literature in the treatment of sudden deafness, however it comes presenting resulted satisfactory in studies carried through for the world. The reply to the treatment she is expressive when the loss is light, in young adult patient e loss in intermediate frequencies. The presented case demonstrates the efficiency of the intratympanic treatment with corticoid in a patient with severe sensorineural loss, involving all the frequencies of a conventional audiometry.
