OBJECTIVE: The salivary glands are the home of many diseases, inflammatory, lithiasic, neoplastic, autoimmune diseases, or congenital anomalies. Closely related to these processes are related fistulas salivary glands, which are rare and their incidence may have several complications. This issue is seldom discussed in the literature. The aim of this paper is to describe a case of surgical treatment of fistula of salivary gland and discuss its possible causes and treatment. CASE REPORT: Female patient, 89 years old, complaining of a discharge outlet orifice of the skin in the right submandibular region, which started a month ago. On examination, it was possible to visualize output aspect salivary secretion of the skin in the submandibullary region without evidence of purulent or inflammatory signals important places. Opted for surgery, which consisted in the removal of the salivary gland by external technical and removing the traditional skin fistula. On the 7th postoperative day progressed with skin dehiscence in a wound is necessary to perform skin flap rotation. The patient showed good healing of the wound and was discharged. The gland was sent to pathology which showed commitment by moderately differentiated squamous cell carcinoma, keratinizing and nonspecific chronic sialoadenitis. CONCLUSION: In general, for patients aged over 60 years old, which show no obvious causes for the appearance of a salivary fistula is the best treatment option to remove with subsequent pathology to exclude neoplastic disease.
