THE CHALLENGE OF DIAGNOSIS IN PEDIATRIC AUDIOLOGY: A CASE REPORT
O DESAFIO DO DIAGNÓSTICO EM AUDIOLOGIA PEDIÁTRICA: RELATO DE CASO

Abstract:

OBJECTIVE: To compare the results obtained in the audiological evaluation through tests evoked auditory brainstem response (PEATE), auditory evoked potential steady state (PEAEE) and visual reinforcement audiometry (VRA), a boy of 5 years and 7 months with cerebral palsy due to neonatal anoxia, served in HUSM by a Hearing Health Program. CASE REPORT: The PEATE has been obtained with monaural and binaural stimulation intensity 90 dBSPL and click. The PEAEE was carried out both ears, the carrier frequencies of 0.5, 1, 2 and 4 kHz modulated at 77, 85, 93 and 101Hz in the left ear and 79, 87, 95 and 103Hz in the OD. The VRA thresholds were obtained using warble tone stimuli. The results of PEATE showed only the presence of wave I of Jewett. The PEATE responses were absent in the maximum intensity of the equipment. In the VRA was obtained responses in 35dBNA 500 Hz, 40 dBNA in 1000 Hz and 2000 Hz in 35dBNA. The speech recognition thresholds were approximately 30 dB HL in the right ear and 25 dBHL in the left ear. CONCLUSION: There is a direct correlation between the results obtained on PEATE, PEAEE and VRA: electrophysiological assessment revealed a bilateral dysfunction of the brainstem, it is impossible to estimate the electrophysiological threshold due to the absence of wave V; ASSR responses were also absent. The thresholds obtained in the VRA reveal the importance and necessity of realization of the subjective evaluation, bringing up the challenge of child audiological diagnosis.
