Introduction: Pressure to intraocular high is the main factor of risk for glaucoma. Hypertensive peaks and hypoxia are contributing factors, both gifts in the SAOS. Objectives: To evaluate peaks and fluctuation of the pressure to intraocular (PIOUS) 24-hour in patients with SAOS. Methods: Patients with polysomnographic diagnosis of SAOS had been invited to carry through measured of PIOUS 24-hour. SAOS was considered with an apnea-hypopnea index (IAH) > 5 n/h. The PIOUS one was measured with tonometer of Goldman 6h, 9h, 12h, 15h, 18h, 21h and 24h, being the peak the biggest found value, and fluctuation the difference between the greater and minor value of the PIOUS one. The results had been described in the measured and comparative shunting line standard and with a group of adults without respiratory complaints through the tests of Qui-square and Mann Whitney, adopting a level of significance of 5%. Results: 26 carriers of SAOS and 169 controls had been enclosed, with age measured of 53+-12 years and 54 +-18 years, respectively. In group SAOS it had predominance of the masculine sex (17), of the group has in contrast controlled composition for 102 women. The averages of PIOUS had shown 18,54 + - 4,86 mmHg in group SAOS versus 15,32 + - 2,64 mmHg (p<0,001), with the peak highest in the schedule of 6h. Similar difference was found for fluctuation with 7,42+-3,59 mmHg in group SAOS versus 5,22+- 2.21 mmHg in the group has controlled (p<0,001). Conclusion: PIOUS and fluctuation more is raised in patients with SAOS, being able, of this form, to be a factor of risk for the development of glaucoma.
