Abstract: |
OBJECTIVES: Followed congenital meatal Atresia of cholestatome rare is told and particularly of difficult diagnosis. We will rare tell a case of a patient with bad formations of external and average ear that evolved with meningitis picture as one complication cholesteatomatous. STORY OF THE CASE: Patient of 24 years, carrier of aural atresia bilateral classroom D of Schucneckt, was light victim of TCE and after 3 weeks of the trauma she evolved with intense chronic headache and vomits being interned by bacterial meningitis. In the image examination one observed thickening of meningis in the RNM and signals of infection in left ear. In the TC of ear complex bad formation of pavilion (microtia) associate the bilateral severe aural atresia with hyper developed average ear and chain to ossicular rudimentary bilateral. In OE it the same presented signals of image with density of soft parts in the atresic plate with erosion of tégmen timpani of side and puncture of propagation of the infection for the possible SNC with fistula liquoric associate. As surgical treatment we opt to the technique of subtotal petrosectomy, described for professor Ugo Fisch, with the intention of exclusion of the average ear of the affected side, exoneration of all the aerates cells, ocluindo the cavity with abdominal fat and remnant of the secular muscle determining the closing of the place of communication and liquoric fistula. CONCLUSION: Ahead of a patient with otologic bad formations, who if presents with meningitis picture, we must have in mind that one cholesteatomatous can have developed although the scarcity of signals and classic otologic symptoms.
