OBJECTIVE: To describe a rare case of chondroblastoma of the temporal bone locally aggressive. CASE REPORT: Male patient, 46, appeared for evaluation with a lesion of fibrous consistency is externalizing the left external auditory channel realized 5 years ago. Symptoms included, besides mixed hearing loss, purulent discharge from fistula in retro-auricular region and progressive facial paralysis at the time, House-Brackmann 4. For 10 years had undergone excision of a similar lesion, chondroblastoma diagnosed by histopathology. He underwent complete resection of the lesion with a margin of safety, with sacrifice of facial nerve and ear. The reconstruction was performed using the latissimus dorsi muscle flap and skin of the dorsal region. Histopathological examination confirmed that a recurrence of chondroblastoma. CONCLUSION: Chondroblastomas temporal bone can be locally aggressive and recurrent. For this reason, today, complete resection is indicated, even if it involves functional impairment.
