TREPHINATION AND THE USE OF THE FRONTAL SINUS ADAPTED TO THE BRAZILIAN REALITY

Abstract:

OBJECTIVE: The nasofrontal duct obstruction is a major challenge as to the treatment. The present work aims to promote the maintenance of trephination and cannula in diseases of the frontal sinus, using materials and equipment available to the Brazilian otolaryngologist. MATERIALS AND METHODS: Three patients with infectious and chronic inflammatory disease of the frontal sinus (nasal polyps and sinusitis), underwent frontal sinus trephination. For such, performed incision in the medial eyebrow and drilling of the anterior wall of frontal sinus (drill diamond and maintained 2 mm. Introduced cannula (needle epidural 1.6 mm x 20 mm) after held suprabullar ethmoidectomy for frontal nasal duct. Through access to irrigation with saline (not used fluoroscein), the tube was allowed to identify the duct. The cannula was secured with suture and maintained for 10 days. The patient was instructed to irrigate through the cannula, 10 ml of SF 3 times a day. RESULTS: Patients were followed for 3 months and all showed maintenance of the permeability nasofrontal duct. CONCLUSION: This procedure using material readily accessible to the otolaryngologist can be extremely useful, benefiting countless patients.
