Background: Osteitis temporal and adjacent bones, also called otitis external is a necrotizing infection of otologic origin with potential impairment of the patient's life, first described in 1959. The aim of this paper is to report a case of necrotizing otitis external requiring chronic use of ciprofloxacin even after cure criteria to take Gallium scintigraphy. CASE REPORT: Male patient, 98 years old, black, insulin-dependent diabetic, said of otalgia in the left ear of strong intensity was 02 months, accompanied by othorrea. He was treated during nearly two months with standard treatment for otitis external, when was suspected diagnosis of necrotizing otitis external, confirmed after Computerizes Tomography and pathological mastoidectomy excluding malignancy. After conservative treatment with standard dose ciprofloxacin for six weeks, the patient improved substantially, but even with taking normal gallium scintigraphy, the patient condition worsened when attempted discontinuation of the antibiotic. It is in use for over a year, with clinical control.
