Abstract: |
OBJECTIVE: case report on the reconstruction of the nasal support with autologous use of graft of rib cartilage after resection of nasal tumor. CASE REPORT: Patient with 37 years of age, masculine sex, white, carried through biopsy of injury suspicion of neoplasias in septal mucous. Carcinoma was diagnosised cystic adenoid. The surgical procedure for ample resection of the tumor was submitted. After 9 months, the patient carried through the first surgical procedure for reconstruction of the defect of nasal sustentation. The graft of the rib cartilage (6th rib) with preservation of perichondrium of thoracic surgery was removed for the team. After, incision was carried through to columella in "V " and detachment of the skin of the nasal back. Wire of Kirschner of 0,8mm in the costal cartilage for the back and of 1,0mm for columella was used. Exertion was located and sutured. It was confectioned, also, graft of "Sheen". After 40 days, new surgical procedure for correction of small deformities was carried through, with the use of graft of the cartilage of the right ear. CONCLUSION: The reconstruction of the nasal structure can need alternative sources of graft, mainly in the cases where the cartilages septal and auricular will not supply the support necessary to stabilize the nasal back. (1,2) The costal cartilage has been the preferred one for the majority of the surgeons, therefore offers an abundant amount of graft and is more trustworthy than the septal cartilage when if it searches support.(2) Moreover, the costal cartilage can easily be incised, keeps the thickness throughout the time and is easily passively of reviewing.(4)
